From the email notification that forms have been sent to your team, click ‘VIEW AND ACTION MHA FORM’ and log in.
You can view the form by going to your notifications, going to your caseload, or searching the patient.
The H3 form can be begun once the application has been done. If you have a paper application, this can should be uploaded using the guide below before the H3 form is completed.
Click ‘start form,’ next to Record of Detention.
Complete the form and click ‘sign'. Review the PDF, once and sign when completed to add your signature. Once signed it cannot be retrospectively changed unless part of de-minimus errors or S15 amends.
Send the completed forms to your Mental Health Law team. You can send these forms to anyone, even if they do not have an account.
You can then review the form and its progress in the activity log which will be available in your caseload.
