How to complete an H3 when the section papers are not completed on Thalamos
If you do not have necessary section papers, you can search for the patient on RiO or create the patient in eMHA by Thalamos, to request any forms to be completed.
If you have received all appropriate documentation not on eMHA by Thalamos, you can click ‘start form, from the Hospital Form options.
Choose the correct hospital form to complete.
Complete the form and click ‘sign'. Review the PDF, once and sign when completed to add your signature. Once signed it cannot be retrospectively changed unless part of de-minimus errors or S15 amends.
Send the completed forms to your Mental Health Law team. You can send these forms to anyone, even if they do not have an account.
You can then review the form and its progress in the activity log which will be available in your caseload
