How to complete an application if you have received both medical recommendations
This following sections will cover how you will receive medical recommendations. This sections will cover the scenario for if you were to complete an application and receive both medical recommendations:
If you have been sent a medical recommendation you can view it by clicking the link in the email notification you have been sent, or by clicking the notification on eMHA by Thalamos.
The form will also be in your team caseload. To view the form, click the form name in the caseload.
You will have the option to ‘accept’ or ‘decline’ the medical recommendation.
If you have already received all medical recommendations you can begin your application by clicking ‘start form’ next to ‘Application’ and selecting an application form.
You always have the option to indicate you will not be making an application by clicking ‘No Application’ and selecting a reason for this from the dropdown list.
Select the medical recommendation(s) for the application. eMHA by Thalamos will check that these medical recommendations are valid.
Complete the form and click 'sign’ to preview the completed form.
If you need to edit the form click ‘go back', if you are ready to sign the form click ‘sign’.
Your signature will be populated from your profile. Confirm this signature will be the electronic representation of my signature, and then click ‘confirm’.
You can now send the application to a recipient. You will be able to view the form and track its progress from your caseload.
