How to complete a s23 Discharge Form.

Before you begin a form you must first establish the patient that you are creating the form for.
  1. If your eMHA is linked to RiO you can:
    1. Search eMHA by NHS number.
      1. or
    2.  Locate the patient on RiO and clickthrough to eMHA
  1. If your eMHA is linked to the Patient Demographic Service (PDS) you can:
    1. Search eMHA by NHS number
      1. or
    2. Search PDS by demographics in eMHA
  2. If your eMHA is not linked to an external system you can:
    1. Search eMHA by patient demographics.


  1. Once you are on the patients eMHA record, you can select the type of form you would like to complete. For a medical recommendation, click the START FORM button next to the Discharge Forms heading

  2. If you will be completing the form, click Start New Form and start typing Section 23 in to the text box. If you are requesting the form from someone else select Invite another user or team to start new form.

  3. Select Discharge of patients: Section 23, then click START FORM.

  4. If there is already a s23 Discharge form in progress for this patient, you will be asked if you would like to start a new form or continue the current one. You can only continue a form if you are the author.

  5. Complete the form as you would complete the paper form, by either selecting the correct option or entering text into the boxes.

  6. Once you have completed the form you can click ‘SIGN’. You will then be able to review the form. If you notice any mistakes, press ‘go backto edit the form.

  7. Complete the form and click SIGN to review the completed form.

  8. ​Once reviewed click SIGN again to add your signature. Ensure all the information is correct before signing.

  9. Once signed you can send the form on to the next person in the pathway. You can send the form to anyone, as long as they have an approved email address (e.g. nhs.net, or .gov.uk) regardless of if they have a Thalamos account.

  10. Once the Send Form box appears, enter the name or email address or the user you are sending the form to and click ADD RECIPIENT.

  11. You can then click send.

  12. The form will appear in your caseload and the team caseload until legal scrutiny has been marked as complete. It will then more to the archive automatically.
