How to request a medical recommendation and complete an application
You can request for a Doctor to complete a medical recommendation through eMHA by Thalamos. You might do this if:
The Doctor does not have a eMHA by Thalamos account set up.
You only have one medical recommendation completed and you would like a Doctor to complete a second one.
You require an Independent s12 Doctor to complete a medical recommendation, and you need to share patient details with them through eMHA by Thalamos.
Access the admission for the person you are assessing in your caseload, or start a new admission.
If you have already been sent one medical recommendation you can ‘accept’ or decline this before progressing.
Click the ‘Request Form’ button next to Request Medical Recommendation and enter the email address of the Doctor you wish to complete a medical recommendation.
They do not have to have an eMHA by Thalamos account to receive this request, however if they do not have an eMHA by Thalamos account, they must have an approved email domain (e.g. NOT hotmail, Google etc.)
The Doctor will receive an email notification with a link to complete the medical recommendation, and once they have completed this form it will be automatically sent back to you.
You can request for as many Doctors to complete medical recommendations as necessary.
Once you have received the required medical recommendations you can click ‘start form’ next to application and complete the relevant application form.
