Receiving SOAD Forms

Receiving SOAD Forms

You will receive SOAD assessments into your eMHA caseloads. This process will be the same as receiving any other type of form via eMHA - You will then be able to share the form on and upload directly into EPRs within eMHA.

When a SOAD sends you a form, you will be able to view it in multiple ways:

1. You will receive an email notification - follow the instructions to view the form.

2. You will also receive a notification in-app. Go to 'Notifications' on the left-hand side of your screen to access the form via the notification.

3. You will also be able to view the form in your 'caseload'

4. You will be able to view and download a PDF version of the SOAD Report Form (SRF) and Statutory Certificates.

5. After accepting and opening the form, it might need to be reconciled into an existing record. As far as possible eMHA does this automatically.

If the NHS number, surname, and date of birth on the SOAD form exactly match those in the existing eMHA patient record, the form will be automatically merged upon accepting the form.
If the NHS number matches but either the surname or date of birth differs, you will be given a suggested merge:

Please note: SOAD forms will NOT affect patients statuses. If there is a need for an MHA status to be changed following a SOAD report this will have to be completed manually by the Mental Health Law team.
