How to Access SOAD Forms on eMHA
Based on feedback from providers this process will not change. You will still receive SOAD forms in the same way:
Step 1: Receive an email from SOAD
You will receive an email from the SOAD to let you know a form has been shared with you.
Step 2: Click on the link
Click on the link provided in the email.
Step 3: Receive a one-time code to your email
A one-time code will be sent to your email - This code will be active for 15 minutes.
Use the code to gain access to the form.
NOTE: If the code expires, please press RESEND GUEST USER ACCESS CODE to receive another code.
Step 5: Download and upload to EPR
Download the form and upload it to your Electronic Patient Record (EPR) system.
