SOAD Provider FAQs

How long will I be able to access SOAD forms? 

You will have access to SOAD forms for 30 days after they have been sent. Please ensure you download all required forms within this period, and store using your local processes. 

What should I do if a SOAD form requires an amend/has an error?

SOADs can amend forms on eMHA. If you require a form to be amended or has an error, you will not be able to request this through eMHA. Please request this using your usual processes. 

How will the one-time access code work - how will users be able to identify which assessment it corresponds to?

When you click the link in the email, it will automatically generate a one-time access code for you to enter. The code is not tied to a specific patient/assessment.

What forms are available for SOADs to complete?

SOADs can complete all treatment forms on eMHA, apart from T1 forms.

1. This has been requested by the CQC, as they are required to be informed prior to this form being completed.
   
2. The number of T1 forms completed by SOADs is very small; however, if you do receive a T1 form, it will not be shared via eMHA. You should continue to receive and process T1 forms using your usual processes.
