Amending Forms in eMHA

eMHA now allows SOADs to amend submitted assessments, helping to correct errors or update information without the need to start a new form.

Amendments can be made to both the SOAD Report Form (SRF) and any associated statutory certificates. This ensures accurate, up-to-date documentation while maintaining clear communication with providers and the CQC.

Once an amendment begins:

  • The original provider will be notified

  • The updated version will be shared once re-signed and re-submitted.

Step-by-Step: Amending a Submitted Form

Step 1: Go to the Archive

Navigate to the Archive page and locate the submitted assessment you wish to amend.

Step 2: Click 'Action' > 'Amend'

Use the Action button beside the assessment and select Amend from the dropdown menu.

Step 3: Confirm Intent to Amend

A banner will appear at the top of the screen.
Review the information and click Continue to confirm you wish to proceed.

Step 4: Review the Form

You will be taken back through each section of the SRF and any associated certificates.

  • Carefully review all content

Step 5: Make Edits

Make the required changes to any part of the form or certificate.

Important: After making your amendments, you will need to sign and re-submit the form again.

Once signed and re-submitted, the amended form:

  • Is automatically shared with the original provider

  • Can be resent to additional recipients if needed

Step 6: Review Amended Documents

  • The original version of the assessment will be marked with a “OLD” watermark
  • Both the original and amended versions will be available to the provider within eMHA
