Once you have completed all six sections of the SOAD Report Form (SRF) and any relevant statutory certificates, the final step is to send the documents to the appropriate provider. This process ensures the SRF and certificates are shared with the care provider, and that all documentation is securely sent to the Care Quality Commission (CQC).
This guide outlines how to submit, sign, and send your completed forms.
The SRF and Statutory Certificates are sent to providers via secure email.
The CQC automatically receives a copy once the forms are signed and submitted.
You may send the forms to multiple recipients if required.
Click the blue “Submit” button on the form page.
If the button appears greyed out, one or more sections may still be incomplete. Complete all fields to enable submission.
Once you click Submit, the system will generate PDF versions of the SRF and any certificates.
Review these documents for accuracy.
Then, click the blue “Sign” button to proceed.
Begin typing the provider’s name in the recipient field.
Select the correct name from the pre-populated dropdown list.
If the provider you are looking for is not listed:
You may enter their email address manually, provided it uses an approved domain (e.g., nhs.net, nhs.uk, gov.uk).
To request a new email be added to the dropdown list, contact:
support@thalamos.co.uk with the provider’s name and email address.
After selecting the provider or entering the email address, click “Add Recipient”.
You may add multiple recipients if necessary (e.g., several team members at a provider).
All added recipients will now appear at the bottom of the 'Sign and Send SOAD Assessment' screen.
Review the list to ensure everyone has been added correctly.
Once all intended recipients are listed, click the blue “Sign” button to complete the process.
Your SRF, Statutory Certificate(s), and any SOAD Notes will now be:
Sent to the selected provider(s)
Automatically shared with the CQC
