Starting a SOAD Assessment in eMHA

To begin a new Second Opinion Appointed Doctor (SOAD) assessment in eMHA, you will need to start by selecting “Begin SOAD Assessment.”

This guide outlines each step for initiating and completing the first part of the SOAD Report Form (SRF), including how to enter patient details and handle NHS number matching.

Step-by-Step: Initiating a SOAD Assessment

Step 1: Enter Case Reference Number

Input the case reference number provided by the Care Quality Commission (CQC).

Step 2: NHS Number Confirmation

You will be asked: Do you have the NHS number for the patient?

  • If yes, select Yes and enter the NHS number in the box provided.

  • If not, select No to proceed without it.

eMHA will search for the patient across two sources:

  • Existing eMHA records

  • The Personal Demographics Service (PDS)

If a Match is Found

You will be alerted if a patient record is found via eMHA or PDS.

Step 4: Confirm the patient

  • Review the patient details displayed

  • If correct, click Select Patient

Step 5: Click Continue to move forward with the assessment

If No Match is Found

If the patient does not exist in eMHA or PDS:

Step 4: You will be alerted that no match was found
Step 5: Click Input Patient Details
Step 6: Manually enter the patient’s information in the required fields

After entering the correct details, click Continue in the bottom-right corner to begin completing the remainder of the SOAD Report Form (SRF).
