Receiving Section 2 or 3 admission forms from the ward

Doctors and AMHPs can complete admission forms for Section 2 or 3 digitally on Thalamos. These can be submitted to either the ward or the Mental Health Act Office to complete the H3.

In the majority of cases, the Mental Health Act Office will receive a complete set of admission forms, including the H3 from the ward. This guide explains how to access and download these forms when the H3 has already been completed by the ward team.

If you need to complete the H3 for a section, please read this article.

  1. You will receive an email from hello@thalamos.co.uk (see below). To access the digital form, please click on the 'View and download MHA form' link and you will be taken to Thalamos Assess.
  2. Note: For security purposes this is an encrypted link which is only valid for 16 days. If you don’t access the link within that time you will need to ask the AMHP to resend the form to you.
  3. You won’t be required to create an account or enter a log in. However, to maintain security you will be asked to enter your name and email address. This should mirror what the sender entered.
  4. Please tick the 'I confirm that I am authorised to view this confidential medical information' box and click 'Login'.


Reviewing the admission forms

  1. The admission forms you have just received are listed in order. You can check you're happy with each form by clicking 'View Form' . You can download each form individually, or you can select the forms and click 'Download as single PDF'
  2. If you aren't happy, you will need to get in touch with the author outside of Thalamos. Section 15 amends should be organised in the same way.
  3.  Don’t forget to log out by clicking 'Close session' when you are done.
