How to search for a patient (if you are not integrated with your local EPR)

This section is relevant if your version of eMHA is not integrated with your local electronic patient record system. 

You can search for a patient to:

  • start a form 
  • see any in progress/completed forms for that patient.

You can search for all patients within your team that have had a form created on eMHA. Depending on your organisation’s data sharing agreement, you might have access to patients outside of your team (e.g. across your ICS). 

  1. Go to  ‘SEARCH PATIENT’ on the left-hand navigation screen. 
  2. Type in the known patient information. You only need to enter the patient’s surname as a minimum to return a potential match.
  3. Click ‘SEARCH’. 
  4. If the patient information you have searched for has multiple matches you will be shown all matches and be shown a warning banner. You will need to select the correct patient and click  ‘CONTINUE’. 
  5. If this is not the correct patient, or you would like to search again, you can click ‘CLEAR SEARCH' and perform a new search. 
  6. If there is no patient match, you can create a patient. Please see how to do this here. 
  7. Once you have selected the correct patient you can start a form. You can also see any in progress forms on eMHA or any form previously completed for the patient on eMHA by someone in your organisation.
