How to create a patient

Before you start a form, you first need to establish which patient you are creating the form for. You can search for a patient if someone in your team has already created a form for them on eMHA by Thalamos. If you need to start a form for a new patient or one which no one has created a form for yet, you can ‘create a patient’.

If you need to start a form for a new patient or one which no one has created a form for yet, you can create a patient.

1. Go to ‘SEARCH PATIENT’ and select ‘CREATE PATIENT’. 
2. Enter the patient’s information. You must complete all fields to create the patient. If you do not know the patient’s date of birth you can select ‘No’. 
3. Once you create a patient, everyone within your organisation will be able to search for this patient.
