Simply click 'Forgotten password' on the log-in screen and you will be directed to enter your email address so that a reset password link can be sent to you. If you don’t receive this email, please check your junk/spam folder.
Only you have access to the information you enter into Thalamos, you decide who to share this information with. The Thalamos team do not have access to anything other than your professional details, so we can make sure only registered professionals are using the service.
All information on the platform is encrypted at rest and in transit. So you can coordinate your assessment safe in the knowledge that your forms are secure.

Yes. Once complete the forms are identical to the statutory forms, just a digital version.
When completing a form, you might notice that the forms aren’t quite identical to the pink ones you are used to completing. This is because they have been simplified to make them less clunky, quicker and easier for you to complete.
A doctor can only send a form to named AMHP. We understand that you don’t always know which AMHP will make the application. We are working on additional functionality to allow AMHPs to send medical recommendations they’ve received to each other. If you need to, you can always download and print a form from Thalamos.
Thalamos is a web application, so you won’t find it on an App Store. This is for security and IG reasons. It is possible to save it to your home screen however, like you would a normal app.

Forms will be sent from within the Thalamos system directly to nursing colleagues and MHA Admin colleagues. Your local policy guidance will explain which hospitals are included in the pilot, if you need a copy, please get in touch.
No – currently all forms must be downloaded and attached to local record systems.
All your completed forms are stored in the Caseload section.
Click on 'Account Settings' at the top right of any page, then click 'Profile'. You will see a green button saying 'Change Password'.
You can send forms to any approved email address (e.g. nhs.net, .gov etc.) regardless of if they have an account with Thalamos or not. They will receive an email notification with an encrypted link. To access the form they will need to input their name and the email address the form was sent to.
On Thalamos, you can send these forms to the acute hospital and they will receive the link which allow them to access, action and download forms.
We have made the process as intuitive as possible so that anyone would be able to follow the steps. If you do not feel comfortable sending the forms to the acute hospitals, you can always download the form and send via alternative methods.
Once the application, joint med rec and H3 has been sent to the MHA office, you will be able to view the H3 form in Thalamos. Under your caseload, you should see a section which says returned forms, within this section, you will be able to view and download the H3 forms once they have been completed and sent to the MHA office
The agreed process is that doctors should send their medical recommendations to the AMHP team email so they will appear in the team caseload. An an email will be to all individuals in the team email address to notify them. We have done this, so that no forms are missed. If you do not want to see these emails, you can set up rules on your email so they will not show up and then you can just search for the form via Thalamos. The AMHP can then take the form from ‘team caseload’ to ‘my caseload’ and the rest of the team will be able to see that the form has been taken.
If the form is sent to an individual AMHP, it will come through to their email and their individual caseload, not the team caseload.
When sending a CTO, the doctor must send the form to an individual AMHP, rather than AMHP team.
We are also working on changes to the software so in the future there will be more flexibility to send to team email address.
You can access Thalamos and update your profile at any time, from any device. Just go to account settings, profile and scroll down to ‘Approved by Local Authority.’ Here you can update it, as you see fit.
You cannot send the form to multiple people at once, but you will be able to send it once, go back to your caseload and resubmit the form, however many times you would like.
