Transferring Incidents Between Officers

You can transfer a Section 136 incident form to another officer in your force any time after it has been created and triaged, and before it has been ended. 

If you have not been transferred a form, supervisors able to transfer forms on behalf of officers by logging into MHForms in Supervisor View. Please contact them in the first instance.

Key Transfer Rules

  • A form can be transferred multiple times, but only to one officer at a time.

  • The Assigned Officer (the officer responsible for the form) is updated only once the transfer is accepted.

  • While a transfer is pending, the form remains editable by the officer who initiated the transfer.

How to Transfer a Form

Step 1: Open the Section 136 incident form
Step 2: Click the ‘Transfer Form’ button
Step 3: Select the officer to transfer the form to
Step 4: Confirm the transfer request

Once accepted by the receiving officer, the form will be reassigned to them as the Assigned Officer.

How They'll Be Notified

When an incident is transferred to another Officer, they will receive a pop-up alert within MH Forms and an email notification. 

Accessing and Responding to the Transfer

Step 1: Log in to MH Forms

Step 2: Respond to the Transfer

You will be prompted to either:

Accept & View
  • This confirms you are taking responsibility of the form and becoming the responsible officer

  • The incident will move to the ‘My Incidents’ section on your homepage

Reject Transfer

  • Decline the transfer if you are unable to take responsibility

  • The form will return to the original officer
