Starting a New 136 Incident Form

Step 1: From the MH Forms homepage, click “New Form”
This will begin a new Section 136 incident form.

Understanding the Form Sections

The Section 136 form is structured into four sections that guide you through logging, recording, and preparing the handover of a detained individual.

Section 1: Create an Incident & Conduct Triage

Log the initial details of the incident and complete a triage to assess the situation.

Section 2: Enter Person Details

Enter key information about the individual, including identifying details and demographics.

Section 3: Record Detention Details

Capture the legal basis for the detention and the surrounding circumstances. This section ensures legal compliance.

Section 4: Provide Key Information for Healthcare Professionals

Record any relevant health or risk information that healthcare teams will need at the place of safety.

Finalising and Sharing

  • Once the above four sections are complete, you will then be prompted to add the place(s) of safety you visited.

Important Notes

  • All four sections remain editable, and the PDF is updated with new information until the form is ended or finished. 

  • You can edit any triage responses except the final question in Section 1: “What action will you take?”
