Sharing a Section 136 Incident with a Healthcare Provider

Once you’ve completed the digital Section 136 incident form, the next step is to share it with the healthcare provider at the last place of safety. This allows them to complete their section of the process and ensures a full, lawful handover under the Mental Health Act.

How to Share the Form

There are two ways to share the digital 136 incident form with the healthcare staff you are speaking to.

Share by Email

Step 1: Click ‘Share’ under 'Share with POS'

You will be able to review the PDF of the form and confirm all the details are correct.

Step 2: Click 'Share by Email'

For some places of safety, email addresses may be pre-populated. When this is the case, these have been verified by the NHS as their preferred emails for receiving s136 Forms.

Type the email address of the healthcare provider you handed over to, press the green plus button, then click ‘Next’.

Always confirm the
email address with the staff you are speaking to before sending. You can enter in multiple email addresses, such as a team email and an individual's email.

Step 3: Confirmation
A banner will appear in the top-right corner confirming that the form has been successfully shared. Click 'Close' and wait for health services to inform ongoing police presence.

Share Without Email

If you are unable to receive an email from the staff you are speaking to or they do not wish to provide one, then you can share an incident code with them to securely access the form themselves.

Step 1: Click 'Share Without Email'

Step 2: Direct healthcare providers to go to: https://emha.thalamos.co.uk/mhf

Step 3: Share the incident code provided on screen. This can also be found on the PDF of the form.

Step 4: Tick the box for "Health services successfully received the the form" when they have successfully accessed the form and click 'Finish'.

Click 'Close' and wait for health services to inform ongoing police presence.
