Sharing a form as an AMHP

You can securely share a single form or a bundle of forms with the hospital or with colleagues by selecting the tick box(es) on the left-hand side and clicking 'Submit form'.


You can enter a message (optional) to accompany your form; be careful not to include any patient data in this message as this will appear in the body of the automatic email sent out. 

Begin to type in the 'Name of Recipient' and a dropdown box should appear. Select the name of the ward or recipient that you wish to send the forms to and you will see the email address will auto-populate.

Once you’ve sent the form, the status in your Caseload will change to submitted.

You will receive a copy of the H3 once an H3 has been completed by the hospital for the forms you submitted. You will be alerted by email and the H3 will appear in the 'Returned Forms' section of your caseload.
