If the person you share the form with does not have a Thalamos account, they will receive an email with an encrypted link.
Note: The link will only be valid for 16 days for security reasons. There will be no personal details about the patient in the body of the email, but it will show them a preview of any optional message you have included.
When the recipient of the email clicks on the link, they will need to confirm their name and email ID for audit purposes. They will also have to tick a box to confirm they are allowed access to the form.
Note: The AMHP will need to use the same name and email address that you have submitted for them, in order to access the form.
They can click to view the form. Once they have checked they are happy with the form they can then choose to download or print it and attach it to the local record system. If they are not happy with the information they will ask you to complete and send them a new form.
They can access the form more than once within the 16 day timeframe, if they need to review and come back to it.
