Sending a form to an AMHP with a Thalamos account as a Doctor

If you send the form to an AMHP with a Thalamos account, the form will automatically appear in their caseload so that they can complete their section and send the completed form to the hospital. They will also receive an email notification.

Note: If there is a mistake on the form you submit, the AMHP will ask you to send them a new admission form.
