Thalamos are a software company that digitise mental health pathways for Mental Health Providers and Police Services. We work with a number of NHS Mental Health Trusts across the country and the Metropolitan Police Service in London.
Thalamos has worked with BTP to digitise their 136 form. MHForms is the new way to manage Section 136 incidents across the force - enabling officers to:
- quickly fill out reports digitally - which streamlines the process for quicker and safer mental health assessments.
- complete only one form to handover to all Places of Safety and A&Es in a secure and easy way
- smoothly transfer incidents between shifts
- have the entire pathway be an auditable process.
All the statutory information on the 136 form and all processes will remain the same.
BTP officers will now record on a digital form rather than a paper one, and communication between officers and NHS staff will still be expected to take place.
Key Programme Dates:
From 19th May 2025: Pilot of the digital form in Manchester and Liverpool to gather initial feedback.
End of July 2025: National deployment across all BTP divisions is scheduled for the end of July.

Engaging with the NHS:
We have created a resource hub for NHS Trusts, explaining the programme to them and providing resources that you can use to communicate with them. If they do not wish to receive forms digitally, either by email or a share-code, then you will always be able to indicate this.

- Any technical issues or questions related to accessing or using the digital 136 form software will be handled by BTP's technology service desk: Support : The Hub
- For questions or issues related to the 136 process, procedures, or application of the Mental Health Act, those will be directed to BTP's integrated mental health team in the control room.
