NHS Staff Guide: How to access and download a digital 5-6

To access a Digital 5-6 you will need the following information from the Place of Safety staff: 

  1. Click on the URL and Enter your email address, the access code & site code
  2. Click sign in as a guest. 
  3. You will then be prompted to review the PDF.
  4. You can download the document by clicking on ‘Download’ in the bottom right corner for your own records.
