How to upload a paper form to Thalamos

You can upload a form not completed on eMHA by Thalamos for an admission on eMHA by Thalamos. You can upload a form using the Form Upload function on either a computer or mobile device.You will only be able to upload a medical recommendation or an application.

There are two ways to access the admission for the patient:

a.  The first is to locate the relevant admission on the team caseload and click the ‘Admission’.
b.  If you cannot see the admission in your caseload you can search for the patient and access the admission. 

If you would like to upload a form for a patient that does not have an admission started for them on Thalamos:

  1. Search for a patient by name (Or create them if they do not yet exist in the system), we have a help guide on how to do this here.
  2. Select the patient that you need to upload a form for and click continue.
  3. When you open their patient records, select admission forms, and click ‘START FORM’
  4. If they have previous forms in this section, you can either click into the relevant admission application, or scroll to the bottom and select ‘START NEW’

  1. Once you have accessed the relevant admission you can click the ‘UPLOAD FORM’ button.

  2. Type in the text box the name of the form that you are uploading, select the relevant form and click 'CONFIRM’

You can upload the following forms: A1, A2, A3, A4, A5, A6, A7, A8, A9, A10, A11

  1. This will open the page where you can select a file from your computer to be uploaded to the Thalamos system.

  1. Click ‘SELECT A FILE’ and pick the form you are uploading.

  1. Files must be PDF or JPG/PNG format, to convert a Microsoft Word document to a PDF:
  2. Open the file in Microsoft Word
  3. Select ‘File’ -> ‘Save As’
  4. Give the file a name, then select the ‘File Format’ dropdown list.
  5. Select PDF and click ‘Save’
  1. Confirm all the details of the form being uploaded and fill in any relevant fields. An uploaded form cannot be amended under section 15 of the Act due to lack of accurate audit trail and complete data record.

  1. When you are happy that everything is correct you can select ‘UPLOAD FORM’, This will open a final preview of the document being uploaded. At this stage do a final check to make sure that everything is legible and correct before selecting ‘UPLOAD FORM’ again.

  1. You will see a pop-up appear asking you to confirm your action of uploading the form, once you slick ‘CONFIRM’ you will get a confirmation pop-up letting you know that the form was uploaded successfully.

  1. You can then view and action the form within the patients’ records. The form will keep a record of when it was uploaded to the Thalamos system


How to upload a form to Thalamos using a mobile device.

  1. Follow steps 1 to 4 from above in this webpage.

  2. When you click the ‘SELECT A FILE’ button your mobile devices camera app will open. (If this is the first time you have uploaded using this device you may have to allow your browser access to your camera)

  3.  Use your mobile devices camera to take a high-quality image of each page of the form.
