How to complete and send forms

  • Before you begin a from you must establish who the patient if that you are creating the form for. There are two ways to do this, you can either search for a patient or go to the patient from your caseload. Please see our help guide on how to do this.

  • Once you have searched for a patient, you can select the type of form that you would like to complete. Forms are categorised into the following types:

    • Discharge Forms: s23, Discharge from Holding Powers

    • Hospital Forms: H1, H2, H5, H6

    • Treatment Forms: T2

  • You can then complete the form as normal. The layout and content of the forms are exactly the same as the paper forms.

  • Once you have completed the form, or the form part you can send it on.

    • Enter the name or email of the recipient

    • select the name from the dropdown list.

    • if recipient is apart of multiple a drop down box will appear select the appropriate team

    • add recipient. repeat if sending to multiple recipient

    • once done, click send.

    • The form will remain in ‘your caseload’ and the ‘team caseload’ where you can see the form progress. Once the MHA Office has marked legal scrutiny as complete, the form will move from the caseloads to the ‘archive’.
