If you spot an error on a medical recommendation or application that you have signed, you can redo the form without having to enter the same details again. You can also request another person to redo their form if you spot an error.
You can only redo a medical recommendation or application before the H3 has been signed.
When a form is redone:
- A new copy of the form opens to correct and sign
- The author corrects and signs it, creating a new form with a new signature
- The original is kept, marked void, and stored in the Voided forms section for audit
- Any form which relies on the redone form, such as an application, will also need to be redone
Before you sign
MHA forms are legal documents. Most redos can be avoided with a careful check before signing:
- Read the full form preview, not just the fields you filled in
- Check patient details, names, addresses, and dates
- Follow all regular scrutiny processes
- Only click 'Sign' once you are sure the form is correct
If you spot an error on a form completed by someone else, you can ask them to redo it:
- Click 'Action' next to the form
- Select 'Request redo'
- Select a reason and add any details
- Click 'Send request'
2. Accept a redo request
If someone has asked you to redo your form:
- Click 'Action' next to the form
- Select 'Respond to redo'
- Update the form
- Click 'Sign'
Once the new form is signed, the original is voided. Voided forms are watermarked and kept below completed forms. Switch off 'Hide forms' to view, download, or send them.
3. Decline a redo request
If you do not agree that the form needs to be redone:
- Click 'Action' next to the form
- Select 'Respond to redo'
- Click 'Decline'
- Enter a reason
- Click 'Confirm decline'
The form stays as signed, and the person who requested the redo will be emailed.
Only the author of a form can start a redo on it:
- Click 'Action' next to the form
- Select 'Start redo'
- Select a reason
- Click 'Start redo'
- Update the form
- Click 'Sign'
If you redo a medical recommendation after the application has been completed, the AMHP will also need to redo their application.
5. Retract a redo request
You can only retract a request before the author accepts it:
- Click 'Action' next to the form
- Select 'Retract redo request'
- Enter a reason
- Click 'Retract request'
6. Cancel a redo in progress
You can only cancel a redo before the new form is signed:
- Click 'Action' next to the form
- Select 'Cancel redo'
- Select a reason and add any details
- Click 'Cancel redo'
If a medical recommendation in the assessment has been redone, you will also need to redo your application. Forms in the process of being redone cannot be used to progress the admission:
- When 'A form needs to be redone' appears, click 'Continue'
- Reselect the linked forms
- Click 'Sign'
