From the email notification, click 'VIEW & ACTION MHA FORM'.
Enter the access code sent to you in a separate email.
You have the option to accept or decline working on the form.
Once you accept you can download the form as a PDF if you wish.

Send a request a doctor to complete a medical recommendation on eMHA
If you wish to complete your application, click start form & select the type of application. You can select the medical recommendation that you have been sent. If the other medical recommendation has been completed on paper you can indicate this to continue.
Fill in the form as usual. The patient's information will be pre-filled.
Click 'SIGN' to review the form. You can click 'GO BACK' to make any changes.
Click 'SIGN' to add in your signature - you can either draw one or upload an image.
Once completed, you can send it onwards to ward staff.
You can download this form and all other relevant forms in the admission, if needed. Click 'ACTION' and download to do so.
