How to edit patient information

If you notice a patient’s information is incorrect you can update this.
  1. Go to the patient’s page and click ‘ACTION’ on the patient banner at the top right of the page.
  2. Click ‘EDIT PATIENT’
  3. Update the patient’s information, click 'UPDATE PATIENT' once you have ensured the details are correct, click 'CONFIRM'. 
Can I edit patient information on a completed form?
Patient data cannot be edited on a signed form. If it is incorrect, you will have to update the patient information using the steps above and complete the form again.

I am in the middle of a form, can I edit patient information?
If you have started a form, but not signed it,
and noticed the patient information is incorrect, you can update the information, and it will automatically be updated on the form.
