Step 1 – Log into your MHA admin account.

Step 2 – Go to new forms.

Step 3 – Click Start form.

Step 4 – First add the patient’s name.

Step 5 – For hospital detained name you can either add this manually or select a hospital from our drop-down list.

  1. If you select a hospital from the drop down list the address will automatically populate.

Step 6 – For hospital transfer name you do the same and select a hospital from the drop-down list or add this manually.

Step 7 – Your signature and name will populate from your profile.

Step 8 – The date will populate to today, but this can be changed.

Step 9 – Click view form.

Step 10 – You will be presented with a pink H4 with part 1 completed.

Step 11 – 3 options:

  1. To change part 1 click edit form.
  2. To save the form but not send it click save. The H4 will appear in your inbox.
  3. To send the form for part 2 to be completed click save and send.
    1. Add the recipient’s name. you can either select this from our drop-down list or add this manually.
    2. If you select a recipient from our drop-down list the recipients email address will populate. If not please add this manually.
    3. To send to more than one person click add recipient.
    4. Click send.
    5. Once sent this form will appear in your MHA inbox as awaiting review.

The recipient will receive an email from Thalamos to complete part 2.

*Once part 2 has been completed you will receive an email notification and the form will be automatically updated in your inbox.
