Step 1 – Log into your MHA admin account.
Step 2 – Go to new forms.
Step 3 – Click Start form.
Step 4 – First add the patient’s name.
Step 5 – For hospital detained name you can either add this manually or select a hospital from our drop-down list.
- If you select a hospital from the drop down list the address will automatically populate.
Step 6 – For hospital transfer name you do the same and select a hospital from the drop-down list or add this manually.
Step 7 – Your signature and name will populate from your profile.
Step 8 – The date will populate to today, but this can be changed.
Step 9 – Click view form.
Step 10 – You will be presented with a pink H4 with part 1 completed.
Step 11 – 3 options:
- To change part 1 click edit form.
- To save the form but not send it click save. The H4 will appear in your inbox.
- To send the form for part 2 to be completed click save and send.
- Add the recipient’s name. you can either select this from our drop-down list or add this manually.
- If you select a recipient from our drop-down list the recipients email address will populate. If not please add this manually.
- To send to more than one person click add recipient.
- Click send.
- Once sent this form will appear in your MHA inbox as awaiting review.
The recipient will receive an email from Thalamos to complete part 2.
*Once part 2 has been completed you will receive an email notification and the form will be automatically updated in your inbox.
