How to complete an H3 as a guest user

1. From the email notification, click 'VIEW & ACTION MHA FORM'.
2. Enter the access code sent to you in a separate email.
3. You have the option to accept or decline working on the form.
4. Once you accept, you will be taken to the form editor. Fill in the form as usual. The patient's information will be pre-filled.
5. Click 'SIGN' to review the form. You can click 'GO BACK' to make any changes.
6. Click 'SIGN' to add in your signature - you can either draw one or upload an image. 
7. Once completed, you can send it onwards to your local MHA office for scrutiny. The user who requested the form from you will automatically be notified that you have completed the form.
8. You can download this form and all other relevant forms in the admission, if needed. Click 'ACTION' and download to do so.
