How to Complete a Medical Recommendation (Med Rec)

Before you begin a form you must first establish the patient that you are creating the form for.
  1. If your eMHA is linked to RiO you can:
    1. Search eMHA by NHS number.
      1. or
    2.  Locate the patient on RiO and clickthrough to eMHA
  1. If your eMHA is linked to the Patient Demographic Service (PDS) you can:
    1. Search eMHA by NHS number
      1. or
    2. Search PDS by demographics in eMHA
  2. If your eMHA is not linked to an external system you can:
    1. Search eMHA by patient demographics.

  1. Once you are on the patients eMHA record, you can select the type of form you would like to complete. For a medical recommendation, click the START FORM button next to the Admission Forms heading

  2. If an admission is already in progress for this patient you will see a popup outlining the forms that have been created so far, click CONTINUE, or follow the prompts to start a new admission.

  3. You can then click START FORM next to the Medical Recommendation heading and select an option from the dropdown list.

    1. Alternatively you can choose NO RECOMMENDATION to decline to make a recommendation. If you select no recommendation you are required to enter a reason why you have chosen not to make a medical recommendation for the patient.

  4. Complete the form and click SIGN to review the completed form.

  5. Once reviewed click SIGN again to add your signature. Ensure all the information is correct before signing.

  6. Once signed you can send the form on to the next person in the pathway. You can send the form to anyone, as long as they have an approved email address (e.g. nhs.net, or .gov.uk) regardless of if they have a Thalamos account.

  7. Once the Send Form box appears, enter the name or email address or the user you are sending the form to and click ADD RECIPIENT.

  8. You can then click send.

  9. The form will appear in your caseload and the team caseload until legal scrutiny has been marked as complete. It will then more to the archive automatically.

Q: Can I complete and send a joint medical recommendation?

A: Yes, you can select a joint medical recommendation from the drop down list. You can then send the form to the next doctor to complete regardless of if they have a Thalamos account or not. If they do not have a Thalamos account you can enter their email address (must be an approved domain and not gmail or hotmail) and they will receive an guest log in to view and complete a form.


Q: I have been requested to complete a medical recommendation, what do I do?

A: You can be requested to complete a medical recommendation, usually by an AMHP. You will receive an email notification and an in-app notification. Click on either of the notifications, and you will see any forms already completed for this admission and then have the option to select which type of medical recommendation you want to complete. You can also indicate you will not be making a medical recommendation.
