The MCA form on MH Forms is used when police are supporting a person under the Mental Capacity Act and taking them to an Emergency Department for care or assessment.
There is no Police Presence Log on an MCA form. Instead, you will record the key incident details, complete the MCA information, record the Emergency Department handover, and enter the name of the NHS staff member receiving the person who is subject to the Mental Capacity Act.

Step 1: Add the latest Emergency Department.
Step 2: Confirm whether police are accompanying the person to the Emergency Department.
Step 3: Search for and select the correct Emergency Department.
Step 4: Confirm the details are correct.
If the Emergency Department cannot be found, enter the details manually.
Step 5: Record the arrival date and time.
Step 6: Confirm the transport method and whether any other Emergency Departments were visited.
Step 7: Confirm the name of the NHS staff member receiving the person who is subject to the Mental Capacity Act.
Step 8: Finish the form.
