How to add a file note to a form
Only MHA Admins will be able to add a file note to forms. You can add a file note to any form which has been sent to you if you are a MHA Admin user.
Choose the form you would like to add a file note to and click ‘menu’
Click ‘add file note'.

Enter any notes you would like to be include
These notes will be saved as a page attachment to the PDF
Click confirm, once you have completed your notes.

Click on ‘View PDF’ to see the file note. The file note will show at the bottom of the form.

The file note will show at the bottom of the PDF once you download.
