How to access a Digital 136 form

  1. You will receive an email notification that you have been sent a digital 136 form to view. 
  2. Click the 'view & action form' button to open the form log in page

  1. You will need to enter the following information 
    1. Your email address 
    2. The access code in the email notification you have received
    3. The Site Code for your Place of Safety 


  1. Click sign in as a guest and you will be logged in and will be able to see the patients name as well as the officers name who shared the form. 
  2. Click on 'Review' to view the 136 form
If you have not received an email notification to access the form, you will still be able to log in: 
  2. Enter you email and the Site Code for your Place of Safety 
  3. Ask the Office who shared the form to provide the access code. It will be on the form progress page under 'Current Place of Safety.
