How do I access a form as a guest?

You don’t need a Thalamos Assess account to review or complete any forms. If you have been sent a form and you do not have a Thalamos account, you are able to access it. 

  1. Click on the link 'view & action MHA form' in the email
  2. Enter your name and the email address that the form was sent to
    1. This email could be your team email. It WILL be the email that the user sent the form to (ensure this email mirrors the spelling that the sender used.)
If you are unable to access this form, you may need the sender to resend the form.
