Ending a Section 136 Incident

Once you have left the Place of Safety (POS) and completed your responsibilities, you must formally end the incident in MH Forms. This final step ensures the record is closed correctly and reflects the outcome of the detention (if known).

How to Finish/End the Incident

If the NHS has engaged:

If the NHS have recorded their request for you to stay and you have responded, or if you are free to leave, then you can finish the incident.

Step 1: Click the green 'FINISH FORM' button in the bottom left corner.

Step 2: Record the date and time the police left. 

Please ensure that you inform staff at the place of safety before leaving.

Step 3: Record if the individual was assessed for their mental health at a Place of Safety.

      a. If they were, you will be asked if you know the outcome of the assessment. 

Step 4: You will be able to download the PDF of the form to share onwards. This will be available in your archive for 3 months. 


If the NHS have not engaged:

If the NHS staff does not engage with the digital form, you must still end the form to access the PDF.

Step 1: Click the red ‘END FORM’ button

This button is located at the bottom of the incident form page.

Step 2: Select the Most Accurate Reason

You will be asked to choose one of five options:

  1. Started in error – The form was created by mistake

  2. Section 136 expired – The 24-hour detention period has ended. You will need select a reason as to why it expired.

  3. NHS organisation did not engage in the digital form – The provider did not accept or complete the form

  4. Medical examination confirmed no mental disorder – A doctor determined the individual is not suffering from a mental disorder

  5. Other – Use if none of the above apply (you may be prompted to provide a reason)

Step 3: You will now be able to download the PDF of the form to share onwards. This will be available in your archive for 3 months.
