To complete a CTO7 using Thalamos, log into your account and begin with 'Start Form'
Use the green narrows to navigate back and forth on the form.
Add the hospital name, address and all other required information. Once you begin to type the hospital name, you should be prompted with a suggestion in the dropdown. When you select a hospital, the address should auto-populate.
Should you not find the hospital/address in this field, you can freely type to override it. If there is anything missing or you have any issues, please email support@thalamos.co.uk.
The information you are inputting is the same as the statutory forms that you're used to. However, instead of crossing out negatives, you're ticking what's applicable in this case.
Follow the steps throughout the form and make sure to tick 'Use my saved signature'. Once happy, select 'View Completed Form'. You can then choose to 'Edit' or 'Save and Send'.
You can then send to the assigned AMHP. Begin typing in their name and the dropdown should prompt and fill in their email address for you.
If unsure of which AMHP to send the form to, please send it to the shared AMHP email address that should be prompted within the dropdown. If there is anything missing or you are experiencing any issues, please email support@thalamos.co.uk.
You will now be able to see this form in 'Your Forms', with the status 'Awaiting Review' until the AMHP has completed their form.
Once the AMHP has returned the form to you, you will get a notification to your email inbox and when you open 'Your Forms' you should see the form there with the status 'Returned Signed.'
Click 'Edit Form' to review it. Once happy, click 'Sign and Complete Form'.
Once you have filled out all relevant details, select 'Save and Send'. You can then begin typing the name of the MHA office and you should find it in the dropdown.

Click 'Send' and the MHA Office will receive an email notification to download the form. The MHA office will not need a log in to access the form. They'll simply have to enter their name and email address that the form was sent to.
