Fill out the details of the hospital and click on the green arrow to navigate to the next section.

Your name will automatically populate if you ticked this option when setting up your profile. You will then need to tick the applicable box(es) as to whether you are a registered medical practitioner or the approved clinician. 


The steps involved reflect the statutory wording on the paper form.

If submitting a form electronically via Thalamos, ensure that you tick the box ‘consigning it to the hospital managers’ internal mail system today at’ and then enter the time.


Then you will be asked to sign the form, you can add your saved signature and the date is pre-populated.

The next part of the form is to be completed by the (person acting on behalf of) hospital manager.

If they are with you in person, Part 2 can be completed on the same device and shared to the MHA Office.

If they are not with you, leave Part 2 blank and use the green ‘Invite Hospital Manager to Review and Sign’ button.

 

Note: If you choose to sign part 2 digitally, you must both have been on the ward and examined the patient.

You can now review Part 1 in the MHA pink style form that you are used to. Click ‘Save and send’ to share the form.

You can enter a message (optional) to accompany your form; be careful not to include any patient data in this message as this will appear in the body of the automatic email sent out. Enter the name of recipient and the recipient’s email address.


When you are ready, click ‘Send’ and an email with an encrypted link will be sent from Thalamos Assess direct to the recipient(s). The recipient must access the link within 16 days.

They will be invited to review Part 1 and complete Part 2 if they are happy. Alternatively, they can ‘Return without Signing’.
