To complete a CTO form, log in to Thalamos Assess and select ‘Section 17A - community treatment order’ from the list in your MHA forms. Click ‘Start form’ to fill in part 1.

The fields mirror the statutory sections of the paper forms, so you should complete all sections as you would normally. Tick boxes replace where you would normally cross an option out. Longer free text boxes are for medical reasoning.
You can navigate between pages by pressing the green arrow buttons on the left and right-hand side of the forms.
Once you have completed all the fields you will be required to add your signature. Hopefully you have already saved your signature when you completed your profile. If so, tick the ‘Use my saved signature’ box and the signature and date will populate.
You can then ‘View Completed Form’ and you will see the information in the MHA pink style form that you are used to.
Review part 1 of the form (you can go back and ‘Edit’ should you wish) and once you are happy click ‘Save and send’ to share your form with an AMHP to complete Part 2.
You can enter a message (optional) to accompany your form; be careful not to include any patient data in this message as this will appear in the body of the automatic email sent out.
Enter the name of recipient and the recipient’s email address.
When you are ready, click ‘Send’ and an email with an encrypted link will be sent from Thalamos Assess direct to the AMHP recipient(s). The AMHP can complete part 2 with or without a Thalamos account. The AMHP recipient must access the link within 16 days.
You will get an email from Thalamos to let you know when the AMHP has completed part 2 of the form. Log in to your Thalamos account, the partially completed form will appear in 'Your Forms'.
You can then 'Edit Form' and if you are happy with it, you can click 'Sign and Complete Form'
Fill out part 3 of the form. Use the 'Save and Send' function to share the form with the relevant MHA Legal Team.
Once you are sure the form has been uploaded to the electronic health record system, you should delete the form from Thalamos.
