After completing the four main sections of a Section 136 incident form, the next step is to record the Place(s) of Safety (POS) where the individual was taken. This ensures a clear, auditable timeline of the incident.
You may need to record multiple places of safety. For example, if the person is first taken to an Emergency Department (ED) and then to a Health-Based Place of Safety (HBPOS).
The final place of safety is the last location visited with the individual.
You must confirm the time the person physically entered the POS before sharing the incident with healthcare providers.
Choose from the drop-down list of known POS locations, or enter the name manually if it's not listed. You will also have to indicate what kind of POS this is, such as an A&E or a 136 suite.
Input the exact date and time the person physically entered the place of safety. If you have not yet arrived, you will be able to indicate this.
Indicate whether there was any delay entering the POS, and if so, explain the reason.
State how you travelled to the POS (e.g. police vehicle, ambulance).
If a place of safety has already been added, you can view or edit its details.
If more than one location was visited (e.g. ED followed by HBPOS), click Yes when prompted to add another POS.
Remember: The final place of safety is the last location the person was taken to and must be recorded accordingly.
