Accessing MH Forms

MH Forms is a web-based application for officers to manage and record Section 136 incidents. MH Forms can be accessed from any device with an internet connection (police desktop, mobile, or tablet)

Accessing MH Forms:

Step 1: Tap the MH Forms icon on your device or visit https://mhf.thalamos.co.uk 

Step 2: Enter your police email address and follow the prompts to log in

Once logged in, you can immediately start or continue any Section 136 form.

How to Access Your Profile:

  • The first time you log in, you'll be prompted to check your details

  • Or, click your name in the bottom-left corner of the screen at any time

If you do not have MHForms as an icon on your device:
  • Open Microsoft Edge on your Police Device.

  • Tap the ⋯ (three dots) menu (usually at the bottom centre or top-right, depending on the device/version).

  • Tap Add to phone or Add to Home screen

  • If you see Install app (or Apps / Install this site as an app), tap that instead - it creates a more app-like icon.

  • When prompted, set the name to: MH Forms

  • Tap Add.

  • If prompted again, confirm Add automatically (or follow the prompt to press and hold the icon and drag it onto the Home screen).
