Sign Up?

It's Simple...

1. Go to https://www.thalamos.co.uk/

2. Click "Sign-Up Now"

3. Enter your Name, Trust Email and GMC No.

You're in!

Create a profile?

Equally as simple...

1. Click "Profile"

2. Fill in the blanks

3. Create a signature (Please note: This is a lot easier on a phone/tablet)

Click save... Done!

Complete a Form?

The forms are very similar to the pink one's you're familiar with

1. Click "Assess"

2. Select Appropriate Forms

3. Follow the steps

Click "View Form" and there it is... the pink form!

Share a Form?

Forms are shared via a time limited encrypted link

1. Click "Save and Send"

2. Enter recipients email address. (Normally an AMHP or MHA Office)

3. Press "Send"

They will receive a message with a link to the form. They can access it with or without an account.
